Send us the information we need to complete our Impact and Matching Assessment and Family Risk Assessment. This will be documents you will already have as part of care planning – we won’t ask you to fill in additional forms for us.
Pre admission / pre assessment visits - We will arrange to come out and meet the family – we really value this part of the preadmission process as it helps to put families at ease and encourages engagement with the service. If a family is moving into one of our residential assessment centres we can arrange a visit for them to look around.
Agree dates for admission / start date for community assessments or support and book in a placement / assessment planning meeting.
If you are working outside of normal working hours - there's a good chance we are too so you can still phone us. We understand that a prompt response is often crucial when making decisions.
